Evidence from randomized controlled trials indicates that hypnosis, relaxation, and meditation techniques can reduce anxiety, particularly that related to stressful situations, such as receiving chemotherapy (see box). They are also effective for insomnia, particularly when the techniques are integrated into a package of cognitive therapy (including, for example, sleep hygiene). A systematic review showed that hypnosis enhances the effects of cognitive behavioral therapy for conditions such as phobia, obesity, and anxiety.
A typical hypnotherapy session has the patient seated comfortably with their feet on the floor and palms on their lap. Of course, the patient could choose to lie down if that option is available and if that will meet the patient's expectation of hypnosis. The therapist can even set the stage for a favorable outcome by asking questions like, "Would you prefer to undergo hypnosis in this chair or on the sofa?" Once patients make the choice, they are in effect agreeing to undergo hypnosis. Depending on the approach used by the therapist, the next events can vary, but generally will involve some form of relaxing the patient. Suggestions will lead the patient to an increasingly relaxed state. The therapist may wish to confirm the depth of trance by performing tests with the patient. For example, the therapist may suggest that when the eyes close that they will become locked and cannot be opened. The therapist then checks for this by having patients try to open their eyes. Following a successful trial showing the patient's inability to open the eyes, the therapist might then further relax them by using deepening techniques. Deepening techniques will vary for each patient and depend largely on whether the patient represents information through auditory, visual, or kinesthetic means. If the patient is more affected by auditory suggestions, the therapist would use comments such as "You hear the gentle patter of rain on the roof;" or, "The sound of the ocean waves allow you to relax more and more." For the visual person, the therapist might use statements such as, "You see the beautiful placid lake, with trees bending slightly with the breeze." Finally, with the kinesthetic person phrases such as, "You feel the warm sun and gentle breeze on your skin," could be used. It is important for the therapist to know if the patient has difficulty with the idea of floating or descending because these are sometimes used to enhance the experience for the patient. However, if the patient has a fear of heights or develops a feeling of oppression with the thought of traveling downward and going deeper and deeper, suggestions implying the unwanted or feared phenomenon will not be taken and can thwart the attempt.
The hypnotized individual appears to heed only the communications of the hypnotist and typically responds in an uncritical, automatic fashion while ignoring all aspects of the environment other than those pointed out by the hypnotist. In a hypnotic state an individual tends to see, feel, smell, and otherwise perceive in accordance with the hypnotist's suggestions, even though these suggestions may be in apparent contradiction to the actual stimuli present in the environment. The effects of hypnosis are not limited to sensory change; even the subject's memory and awareness of self may be altered by suggestion, and the effects of the suggestions may be extended (posthypnotically) into the subject's subsequent waking activity.

It is far easier to describe what hypnosis is not rather than to describe what it is. For example, it is not one person controlling the mind of another. The patient is not unconscious and does not lose control of his or her faculties. People will not do things under hypnosis that they would be unwilling to do otherwise. The person being hypnotized is always in control. The hypnotized person decides how deep the trance will be, what suggestions will be accepted, and when to awaken. Therefore, a hypnotyized person cannot be forever "lost" if the therapist should fall dead during an induction or while the patient is deep in trance.

He also believed that hypnosis was a "partial sleep", meaning that a generalised inhibition of cortical functioning could be encouraged to spread throughout regions of the brain. He observed that the various degrees of hypnosis did not significantly differ physiologically from the waking state and hypnosis depended on insignificant changes of environmental stimuli. Pavlov also suggested that lower-brain-stem mechanisms were involved in hypnotic conditioning.
"How long will I spend in therapy?", is like asking, "How long is a piece of string?" Everyone is different and everyone's individual needs and circumstances vary. There is no definitive answer. However, while some talking therapies can require commitments of a year or more, hypnotherapy tends to be a much faster solution. The average length of time I spend with a client is around 4-6 weekly sessions, to create sustainable changes which some have been trying to implement for years.
Although most practitioners receive their training in hypnotherapy or relaxation as a part of their academic training, the American Society of Clinical Hypnosis and the Society for Clinical and Experimental Hypnosis maintain training programs as well as a registry of practitioners (see previous box). Training in teaching relaxation techniques is provided through various routes from self-teaching and apprenticeships to a number of short courses. Many yoga centers also teach relaxation and offer courses to train yoga teachers.teachers.
However, Freud gradually abandoned hypnotism in favour of psychoanalysis, emphasizing free association and interpretation of the unconscious. Struggling with the great expense of time that psychoanalysis required, Freud later suggested that it might be combined with hypnotic suggestion to hasten the outcome of treatment, but that this would probably weaken the outcome: "It is very probable, too, that the application of our therapy to numbers will compel us to alloy the pure gold of analysis plentifully with the copper of direct [hypnotic] suggestion."
Hypnosis might not be appropriate for a person who has psychotic symptoms, such as hallucinations and delusions, or for someone who is using drugs or alcohol. It should be used for pain control only after a doctor has evaluated the person for any physical disorder that might require medical or surgical treatment. Hypnosis also may be a less effective form of therapy than other more traditional treatments, such as medication, for psychiatric disorders.

Austrian physician, Franz Mesmer (1734–1815), is credited with being the first person to scientifically investigate the idea of hypnotherapy, in 1779, to treat a variety of health conditions. Mesmer studied medicine at the University of Vienna and received his medical degree in 1766. Mesmer is believed to have been the first doctor to understand the relationship of psychological trauma to illness. He induced a trance-like state, which became known as mesmerism, in his patients to successfully treat nervous disorders. These techniques became the foundation for modern-day hypnotherapy.
In the 1980s and 1990s, a moral panic took place in the US fearing Satanic ritual abuse. As part of this, certain books such as The Devil's Disciples stated that some bands, particularly in the musical genre of heavy metal, brainwashed American teenagers with subliminal messages to lure them into the worship of the devil, sexual immorality, murder, and especially suicide. The use of satanic iconography and rhetoric in this genre provokes the parents and society, and also advocate masculine power for an audience, especially on teenagers who were ambivalent of their identity. The counteraction on heavy metal in terms of satanic brainwashing is an evidence that linked to the automatic response theories of musical hypnotism.
Hypnosis is normally preceded by a "hypnotic induction" technique. Traditionally, this was interpreted as a method of putting the subject into a "hypnotic trance"; however, subsequent "nonstate" theorists have viewed it differently, seeing it as a means of heightening client expectation, defining their role, focusing attention, etc. There are several different induction techniques. One of the most influential methods was Braid's "eye-fixation" technique, also known as "Braidism". Many variations of the eye-fixation approach exist, including the induction used in the Stanford Hypnotic Susceptibility Scale (SHSS), the most widely used research tool in the field of hypnotism. Braid's original description of his induction is as follows:
Controversy surrounds the use of hypnotherapy to retrieve memories, especially those from early childhood or (supposed) past-lives. The American Medical Association and the American Psychological Association caution against recovered-memory therapy in cases of alleged childhood trauma, stating that "it is impossible, without corroborative evidence, to distinguish a true memory from a false one." Past life regression, meanwhile, is often viewed with skepticism.
In Trance on Trial, a 1989 text directed at the legal profession, legal scholar Alan W. Scheflin and psychologist Jerrold Lee Shapiro observed that the "deeper" the hypnotism, the more likely a particular characteristic is to appear, and the greater extent to which it is manifested. Scheflin and Shapiro identified 20 separate characteristics that hypnotized subjects might display: "dissociation"; "detachment"; "suggestibility", "ideosensory activity"; "catalepsy"; "ideomotor responsiveness"; "age regression"; "revivification"; "hypermnesia"; "[automatic or suggested] amnesia"; "posthypnotic responses"; "hypnotic analgesia and anesthesia"; "glove anesthesia"; "somnambulism"; "automatic writing"; "time distortion"; "release of inhibitions"; "change in capacity for volitional activity"; "trance logic"; and "effortless imagination".
In conventional hypnosis, you approach the suggestions of the hypnotist, or your own ideas, as if they were reality. If the hypnotist suggests that your tongue has swollen up to twice its size, you'll feel a sensation in your mouth and you may have trouble talking. If the hypnotist suggests that you are drinking a chocolate milkshake, you'll taste the milkshake and feel it cooling your mouth and throat. If the hypnotist suggests that you are afraid, you may feel panicky or start to sweat. But the entire time, you are aware that it's all imaginary. Essentially, you're "playing pretend" on an intense level, as kids do.
Research into hypnosis in military applications is further verified by the Project MKULTRA experiments, also conducted by the CIA. According to Congressional testimony, the CIA experimented with utilizing LSD and hypnosis for mind control. Many of these programs were done domestically and on participants who were not informed of the study's purposes or that they would be given drugs.
But psychiatrists do understand the general characteristics of hypnosis, and they have some model of how it works. It is a trance state characterized by extreme suggestibility, relaxation and heightened imagination. It's not really like sleep, because the subject is alert the whole time. It is most often compared to daydreaming, or the feeling of "losing yourself" in a book or movie. You are fully conscious, but you tune out most of the stimuli around you. You focus intently on the subject at hand, to the near exclusion of any other thought.
Psychiatric nurses in most medical facilities are allowed to administer hypnosis to patients in order to relieve symptoms such as anxiety, arousal, negative behaviours, uncontrollable behaviour, and to improve self-esteem and confidence. This is permitted only when they have been completely trained about their clinical side effects and while under supervision when administering it.
The first neuropsychological theory of hypnotic suggestion was introduced early by James Braid who adopted his friend and colleague William Carpenter's theory of the ideo-motor reflex response to account for the phenomenon of hypnotism. Carpenter had observed from close examination of everyday experience that, under certain circumstances, the mere idea of a muscular movement could be sufficient to produce a reflexive, or automatic, contraction or movement of the muscles involved, albeit in a very small degree. Braid extended Carpenter's theory to encompass the observation that a wide variety of bodily responses besides muscular movement can be thus affected, for example, the idea of sucking a lemon can automatically stimulate salivation, a secretory response. Braid, therefore, adopted the term "ideo-dynamic", meaning "by the power of an idea", to explain a broad range of "psycho-physiological" (mind–body) phenomena. Braid coined the term "mono-ideodynamic" to refer to the theory that hypnotism operates by concentrating attention on a single idea in order to amplify the ideo-dynamic reflex response. Variations of the basic ideo-motor, or ideo-dynamic, theory of suggestion have continued to exercise considerable influence over subsequent theories of hypnosis, including those of Clark L. Hull, Hans Eysenck, and Ernest Rossi. It should be noted that in Victorian psychology the word "idea" encompasses any mental representation, including mental imagery, memories, etc.

Last May , a gentleman residing in Edinburgh, personally unknown to me, who had long resided in India, favored me with a letter expressing his approbation of the views which I had published on the nature and causes of hypnotic and mesmeric phenomena. In corroboration of my views, he referred to what he had previously witnessed in oriental regions, and recommended me to look into the Dabistan, a book lately published, for additional proof to the same effect. On much recommendation I immediately sent for a copy of the Dabistan, in which I found many statements corroborative of the fact, that the eastern saints are all self-hypnotisers, adopting means essentially the same as those which I had recommended for similar purposes.
The weird thing is I was a really shy kid, and would never act like a moron or do embarrassing stuff (like he has us do) in front of my whole school - WILLINGLY! But I DID! I remember feeling really calm, and confident and WANTED to put on a fun show for the school. It felt like the audience was cloudy and obscured by a fog and I just felt so calm and zen like in a trance - but totally aware and intentionally playing along. The last thing he has us do at the end of the event was ask for a signature from anyone we met, believing they were a celebrity (we were instructed to have them sign our hands). I remember people's expressions, they thought we were insane, but i felt empowered by being able to convince others that I was hypnotized - like they were being laughed at not me.
This finding—that PHA temporarily disrupted some people’s ability to recall the past—echoes decades of hypnosis research. What is entirely new in Mendelsohn et al.’s study is their demonstration that PHA was associated with a specific pattern of brain activation. Consistent with what normally occurs in remembering, when people in the non-PHA group performed the recognition task and successfully remembered what happened in the movie, fMRI showed high levels of activity in areas responsible for visualizing scenes (the occipital lobes) and for analyzing verbally presented scenarios (the left temporal lobe). In stark contrast, when people in the PHA group performed the recognition task and failed to remember the content of the movie, fMRI showed little or no activity in these areas. Also, fMRI showed enhanced activity in another area (the prefrontal cortex) responsible for regulating activity in other brain areas.
We know it is hard to sort out all the different claims made by some so-called “hypnosis schools and boards.” Stay away from “distance learning” or correspondence courses that claim to certify you as a clinical hypnotherapist. Hypnosis and hypnotherapy is a healing art based on scientific methods. Only basic hypnotherapy theory can be obtained from the right books or videos. Effectiveness is unlikely without live demonstrations, in-depth and advanced discussion, question and answer opportunities and supervised clinical practice. Just as correspondence courses are inappropriate for counselors, medical doctors and massage therapists, they are wrong for people who want to be effective and successful clinical hypnotherapists.
Confusion can occur when one seeks a hypnotherapist, as a result of the various titles, certifications, and licenses in the field. Many states do not regulate the title "hypnotist" or "hypnotherapist," so care must be exercised when selecting someone to see. As a rule, it is best to consult a professional in the field of mental health or medicine, although alternative sources for hypnosis are available. Care must be taken also by the therapist to ensure adequate training and sufficient experience for rendering this specialized service. The therapist must be well grounded in a psychotherapeutic approach before undertaking the use of hypnotherapy. Professionals should not attempt hypnotherapy with any disorder for which they would not use traditional therapeutic approaches. The patient seeking hypnotherapy is reminded that unskilled or amateur hypnotists can cause harm and should not be consulted for the purpose of implementing positive change in an individual's life. The detrimental effects of being subjected to amateur or inadequately trained persons can be severe and long lasting. (See abnormal results below.)
